The Chronic Care Management program is in particular addressing the reduction in Adverse Drug Events and reducing unnecessary emergency room and hospital visits. This program can best be delivered by a Board Certified Pharmacist, who training is in medication and medication risk management. Our Founder and CEO is a Board Certified Geriatric Pharmacist and has built a team of health care executives and staff to develop and deliver the program.
